The implementation of this managed care process, based on best practise, and delivered by a multidisciplinary healthcare team results in improved outcomes for patients; specifically, a faster recovery time and shorter hospital stay.<\/p>\n<p>The benefits of a comprehensive enhanced recovery programme have been demonstrated in colorectal, endocrine, gynaecological, urological, vascular and orthopaedic surgeries. Several detailed guides are available to surgical teams wishing to implement an Enhanced Recovery programme [1].<\/div><\/div>\n<div class='content-column one_third last_column'><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-273\" src=\"https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/Kehlet-235x300.png\" alt=\"\" width=\"235\" height=\"300\" srcset=\"https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/Kehlet-235x300.png 235w, https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/Kehlet.png 654w\" sizes=\"auto, (max-width: 235px) 85vw, 235px\" \/> <strong>Professor Henrik Kehlet<\/strong>\u00a0<\/div><div class='clear_column'><\/div>\n<p>Six stages of the perioperative process have been identified:<\/p>\n<ol>\n<li>Referral<\/li>\n<li>Pre-operative care by the hospital team<\/li>\n<li>Admission to hospital<\/li>\n<li>Operative care<\/li>\n<li>Post-operative care in the hospital<\/li>\n<li>Follow-up \u2013 rehabilitation and discharge<\/li>\n<\/ol>\n<p>Evidence-based care modalities are combined to create a protocol to markedly decrease postoperative<br \/>\nmorbidity (such as hospital acquired infections and venous thromboembolism), decrease length of hospital stay, and decrease time to resumption of normal daily life.<\/p>\n<p>There are three key evidence-based interventions that when implemented by surgical teams during \u2018operative care\u2019 combine to enhance the recovery of patients, these are:<\/p>\n<ul>\n<li>Minimally invasive surgery: Shorter incisions and laparoscopic surgical techniques reduce the stress of surgery.<\/li>\n<li>Intraoperative fluid management: <a href=\"https:\/\/www.deltex-academy.com\/decision_tree\/stoke-volume-optimisation\/\" target=\"_blank\" rel=\"noopener noreferrer\">Oesophageal Doppler guided fluid management <\/a>has been shown to reduce complications, reduce intensive care admissions and reduce length of hospital stay.<\/li>\n<li>Modern anaesthesia and pain relief: Modern anaesthetics and better pain relief allowing faster postoperative mobilisation and therefore recovery.<\/li>\n<\/ul>\n<p>These individual interventions are more effective when implemented together than alone \u2013 \u2018the sum is greater than the parts\u2019.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone wp-image-274 size-full\" src=\"https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/kehlet-diagram.png\" width=\"862\" height=\"689\" srcset=\"https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/kehlet-diagram.png 862w, https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/kehlet-diagram-300x240.png 300w, https:\/\/www.deltex-academy.com\/decision_tree\/wp-content\/uploads\/2017\/01\/kehlet-diagram-768x614.png 768w\" sizes=\"auto, (max-width: 709px) 85vw, (max-width: 909px) 67vw, (max-width: 1362px) 62vw, 840px\" \/><\/p>\n<p>Scott, Fearon, DOH<\/p>\n<h3><strong><span style=\"color: #003087;\">Reference<\/span><\/strong><\/h3>\n<p><a href=\"http:\/\/www.nhsiq.nhs.uk\/resource-search\/publications\/nhs-imp-fulfilling-the-potential.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">NHS Enhanced Recovery Partnership Programme.
